The process of seeking support for elderly and homebound individuals can be complicated. But with 120 years of experience, LifeCare Alliance provides a reliable and trustworthy solution.
Our team of Central Ohio Resource Experts collaborates with fire departments, police departments, physicians, hospitals, other agencies, families, and individuals. We coordinate care and support individuals who will benefit from a daily wellness check and other services to support them in their homes.
C.O.R.E. Services Offered
LifeCare Alliance can provide a variety of support services, subject to applicant eligibility, or refer clients to the appropriate local resource. Here is a sampling of what we can offer:
- Daily Meals-on-Wheels deliveries and/or daily wellness checks.
- Direct care and personal care assistance.
- In-home visits by a registered dietitian to help with diabetes management.
- Home repairs for clients in need of safety-related and health-related maintenance in owner-occupied residence.
STEP 1: To refer a client for LifeCare Alliance services, complete a referral form. This may be submitted online, or if you prefer, you can download and a printable form. Fax the completed form to 614-278-3143 or send via secure email to firstname.lastname@example.org.
STEP 2: Within 24 hours of receiving the referral form or initial contact, LifeCare Alliance will reach out to the prospective client. If the applicant consents, we will conduct a home visit and evaluation of needs. Services will be scheduled based on the results of the home visit, as well as eligibility requirements of programs and funding sources.
