Leilani Monfort Named Navigator of the Year

The Cancer Support Community of Central Ohio recently named Leilani Monfort, Home Care Support Manager at LifeCare Alliance, as their Navigator of the Year! This award recognizes her work with LifeCare Alliance Cancer Services, where she’s been making a real difference in the lives of the community since 2020.

Leilani’s role as a patient navigator is truly invaluable to the hundreds of clients we serve each year. Every day, she works to break down the barriers that cancer patients face, arranging for medical transportation, medication assistance, helping with housing or utility costs, and connecting patients with countless other resources in the community.

Since taking on her role as Home Care Support Manager, Leilani has worked tirelessly to remove financial and logistical hurdles that could delay treatment. She’s built strong partnerships with providers like The James Cancer Hospital, OhioHealth, and Nationwide Children’s Hospital, ensuring patients get the help they need to focus on their recovery, rather than their challenges.

Leilani’s has personally delivered medications to patients who can’t make it to the pharmacy, made sure patients with language barriers receive the support they need, and much more. Her work shows the true power of compassion and how a health navigator can change lives. Being There Matters!

As the 2024 holiday season is in full swing, we are filled with gratitude for the outpouring of support from our community. Your generosity is truly making a difference in the lives of our clients.

At LifeCare Alliance, we work year-round to support the health and well-being of our clients. During the holidays, your efforts add an extra layer of care and joy for those we serve.

We extend our heartfelt thanks to everyone who has signed up to participate in our Holiday Gift Program. Your kindness, whether in donating gift cards, creating handmade items, delivering gifts, or contributing in other meaningful ways, will brighten the season for so many.

While sign-ups for this year’s program will close on December 2, there is still time to finalize your involvement if you’ve already expressed interest. Alternatively, if you missed the deadline, but still want to give back this holiday season, consider signing up for a Meals-on-Wheels route to deliver nutritious meals and a friendly visit to our clients.

As the end of the year approaches, do you find yourself wondering what impact you made this past season? Or are you looking for ways to give back in the New Year? Well, you can start here by joining our Spirit Society at LifeCare Alliance.

The generosity of our donors enabled our skilled and caring team to reach out to over 30,000 Ohioans and provide over 1.5 million meals last year.  The support of our Spirit Society members ensures the predictable sustainability of this service next year and for years to come.  Our Spirit Society members have shared what makes being a member so special to them:

“It just makes me feel good to support LifeCare Alliance”.

“I continue to believe that LifeCare Allilance offers the best range of services to a wide circle of individuals, doing so in a well-managed and economically sustainable method”.

Our Spirit Society members have the opportunity to join us for special events throughout the year as well as receive quarterly updates from our CEO, Chuck Gehring.

Season’s Greetings,

As we enter the holiday season and reflect on this past year, I can’t help but feel thankful for our generous Alliance of donors and volunteers who support us.

Thanks to you, neighbors throughout Central Ohio can receive Nutrition, Health, and Support services from us, including home delivered meals, free or low-cost cancer screenings, diabetes education, arthritis support, senior pet care, occupational therapy services, and so much more.

You’ve helped us meet the continually growing community need without a waitlist, even with a 98% increase in demand for meals compared to pre-COVID numbers.

In the spirit of the season, will you consider making a year-end gift to LifeCare Alliance? Together, we can ensure that our neighbors facing challenges stemming from medical conditions, disabilities, and aging in place can live with independence and hope.

Lace Up for Camp Hamwi

As part of our diabetes services at LifeCare Alliance, we offer summer camp programs to help children and teens learn to manage their diabetes.

Camp Hamwi, which consists of day camps for children ages 3-7, and overnight camps for youths 13-17, offers a diverse program for kids of all backgrounds. Campers are encouraged to try new skills and explore their individual talents. Activity options include horseback riding, archery, volleyball, soccer, canoeing, swimming, arts and crafts, drama, dance, outdoor living skills, nature crafts, and outdoor adventure. Incorporated into the daily schedule is diabetes education sessions to increase campers’ understanding of diabetes.

The camp environment promotes team building, community, and understanding among peers who all share the experience of managing diabetes. After a long school year, where many feel out of place or isolated, campers are able to make friends and engage in fun just like everyone else.

Even though Summer of 2025 seems a long way off, we are currently getting ready for our next camp season. One of the easiest and fun ways you can support Camp Hamwi is by registering for The Dash for Camp 5K Run/Walk. It’s a family-fun event that makes you feel good. You can register right up to the day before the event. Sorry there’s no on-site registration.

As the 2024 holiday season approaches, we are reminded of the joy and warmth that come with this special time of year—gathering with loved ones, enjoying festive meals, and celebrating together. For many of us, these moments are filled with happiness. However, for some of our clients, this season may be less cheerful.  This year, you can help us bring a bit of holiday magic to those who need it most. Your support can make a significant difference in brightening their season and spreading joy.

At LifeCare Alliance, we provide essential services throughout the year, including home-delivered meals, mammograms, cancer screenings, arthritis support, diabetes education, and more. As we enter the holiday season, we would love to add an extra touch of cheer for our clients. Will you join us in making this possible?

Here are several ways you or your group can participate:

  • Donate Gift Cards: Gift cards from Kroger, Walmart, Target, Meijer, and Speedway in $25 and $50 amounts are always appreciated.
  • Choose from Our Amazon Wish List: See the link below to mail items directly to our office to include in holiday bags
  • Sign Up to Deliver Gifts: Help bring joy directly to our clients’ homes. (This sign- up will be available early December)
  • Sponsor a Family or Individual: Provide a personalized gift packages.
  • Create Decorations or Handmade Items: Share your creative talents.
  • Donate Craft Supplies: Help us gather materials for handmade gifts.

What’s the Difference Between Type 1 and Type 2 Diabetes?

While many people have heard of diabetes, there may be much confusion around the differences between type 1 and type 2 diabetes. To help you understand the main differences, here are a few of the most distinguishing aspects of each type.

Type 1 diabetes is a lifelong autoimmune disorder that is often diagnosed in young people but can develop at any age. Researchers aren’t sure how to prevent it or what triggers it, but type 1 diabetes requires the administration of insulin, typically through an injection or a wearable insulin pump and continuous glucose monitoring system.

Type 2 diabetes occurs when the pancreas does not produce enough insulin or when the body cannot use insulin properly. While researchers are still looking for an exact cause, genetics play a part in risk factors. There is no cure, but it can be controlled through lifestyle and medical interventions. It is most commonly diagnosed in adults, and although the incidence of type 2 diabetes in children is relatively low, it has nearly doubled in the United States since 2003.

Diabetes has become the world’s fastest-growing chronic disease, prompting the CDC to declare it a global epidemic.

Diabetes Awareness Month: Understanding Diabetes

November is American Diabetes Awareness Month, a time to spread awareness, educate, and advocate for those living with diabetes. According to the CDC, more than 38 million Americans have been diagnosed with diabetes. With that being nearly 12% of the U.S. population, the odds are high that you know someone who is living with this disease. November is a great opportunity to learn more and understand the condition.

Diabetes is a chronic disease that occurs when either the pancreas does not produce enough insulin or the body cannot use the insulin it produces. Insulin is a hormone that moves blood glucose (blood sugar) into cells so that it can be converted into energy. Without enough insulin, the blood glucose sits outside the cells, building up in the bloodstream. This causes high blood glucose, which, over time, if left untreated, can lead to life-threatening complications.

Diabetes is one of the leading diagnoses for LifeCare Alliance Meals-on-Wheels clients, so we have a vested interest in educating, counseling, and supporting those clients in managing their disease. In addition, we operate a summer camp program for youths living with diabetes. LifeCare Alliance has a long-standing dedication to helping people of all ages manage their diabetes so that they may live healthier lives.

This month we encourage you to learn all you can about diabetes and how you and your loved ones can live with independence and dignity while battling this disease.
