POSITION:  Help-at-Home Errands Homemaker (Senior Shopper)

PURPOSE:   Help homebound clients maintain independent living by providing errands service.

RESPONSIBLE TO:  Client Services Coordinator, Help-at-Home Director

DUTIES AND RESPONSIBILITIES:
1. Coordinate with client the time and date of visit.
2. Go over grocery and/or medication lists with client.
3. Pick up items and promptly return to client and help put away if appropriate.
4. Complete service record fully with the client and document financial transactions.
5. Return all receipts and grocery lists to client and submit service record to LifeCare Alliance.
6. Report to supervisor any concerns regarding client.

KNOWLEDGE, SKILLS AND ABILITIES:
1. Must be dependable and responsible.
2. Enjoy working with older adults.
3. Must have good documentation skills.
4. Ability to communicate well with clients and staff.
      
TIME REQUIREMENTS: 2 hour visit every other week.  Schedule is flexible and can be worked out   between client and volunteer as to day and time visit is to be made.

QUALIFICATIONS:
1. Valid Drivers License
2. Maintain adequate insurance.
3. Completion of Volunteer Orientation and required documents.

RESPONSIBLE TO:  Client Services Coordinator, Help-at-Home Director.

DUTIES AND RESPONSIBILITIES:
1. Coordinate with client the time and date of visit.
2. Go over grocery and/or medication lists with client.
3. Only complete tasks which are on the service record, any questions regarding service should be addressed to the Client Service Coordinator.

4. Follow all OHSAA quidelines which will be outlined in the volunteer orientation.

5. Complete service record fully with the client and return to LifeCare Alliance.
6. Report to supervisor any concerns regarding client.
