The 2009 Senior farmer's market nutrition program

The Senior Farmers' Market Nutrition Program (SFMNP) allows low-income seniors in Central Ohio to obtain produce from local farmers.

waiting list for franklin county coupons

All available coupons for Franklin County have been distributed.  You must submit a completed application to be on the waiting list.  There are two ways to do so:

  1. In person at LifeCare Alliance's Carrie's Café (670 Harmon Avenue) between 10 am and 2 pm, Monday through Friday

  2. By mail.  Send completed applications to:
    670 Harmon Avenue Columbus, Ohio 43223
    (attention: Barb)

You can download applications below or pick up applications at Carrie's Café. We will establish waiting list in the order in which the coupons are received, and we will utilize postmarks for the mailed applications.  Incomplete applications will not be accepted.  If we are able to distribute more coupons at any point during the season, the individuals on the waiting list will be notified with instructions on how they can receive their coupons.
