Our pioneering approach to community health care continues by providing specialty services to our care recipients, which includes a Visiting Dietitian, Social Workers and Therapists. These services are what separate LifeCare Alliance from our competitors. The certified staff is committed to providing the care needed helping our clients to remain independent with dignity in their own homes.
A qualified licensed social worker can provide assistance to homebound clients by connecting them to community resources such as, Medicare, Medicaid, Food Stamps, Medication Assistance, Transportation, Home Care, Housing and a variety of other benefits or programs. Grief counseling is also available to those who may need or request it.
The Registered Dietitian (RD)will provide in-home assistance to individuals who are current patients in the Visiting Nurse Program. The RD provides Medical Nutrition Therapy for conditions such as diabetes, end stage renal disease (ESRD) and heart disease. MNT involves nutrition assessment and interventions such as education and counseling.
